In this guide
In San Bernardino, a routine check-up means a local practice or the nearest public health post; for a serious emergency, the reference hospital for Cordillera is the Hospital Regional de Caacupé, run by the Ministry of Health (ABC Color (opens in a new tab)). Plan around the season: from December to mid-February the town fills up (Wikipedia (opens in a new tab)) and the access roads jam.
Is it an emergency? What to do first
- Serious emergency (chest pain, trouble breathing, a road or lake accident): 141 is the SEME out-of-hospital medical emergency line and 911 is the police (ITU (opens in a new tab)). SEME's fleet is concentrated in Asunción and Central (Última Hora (opens in a new tab)), so if you can move the patient safely, don't wait for an ambulance.
- Reference hospital: the Hospital Regional de Caacupé is the department's main hospital and takes patients from the departmental capital and nearby towns (ABC Color (opens in a new tab)). During the December pilgrimage it also runs first-aid posts (Última Hora (opens in a new tab)).
- Something that can wait a few hours (fever, a cut, high blood pressure): ask at the health post in your barrio or compañía before making the trip.
Public hospital, IPS or private care
- Public: Ministry of Health services are the option if you have no insurance. Arrive early and expect queues.
- IPS (social security): in Cordillera, the IPS unit in Caacupé runs from a small rented building and the institute admits it lacks space (ABC Color (opens in a new tab)). To book, option 1 on the IPS call centre is appointments (Última Hora (opens in a new tab)).
- Private or prepaid plans: check that a plan is licensed before signing up. In 2024 the Superintendencia de Salud shut six prepaid medical firms in Asunción and Central and removed them from its register (La Nación (opens in a new tab)).
How to choose a doctor or practice
- Name the specialty up front: general medicine, paediatrics, gynaecology, dentistry. They can tell you if someone is in that day.
- Ask how they work: appointment or first-come, first-served, and what time they start handing out numbers.
- Bring everything: ID, insurance card, previous test results and a list of your medicines. Many practices work in Spanish only, so a written summary helps.
- Antibiotics: since November 2018 they are sold only with a prescription valid for five days (La Nación (opens in a new tab)). Get the full prescription before you leave.
If you live in San Ber year-round
Since the pandemic, many people from Asunción have moved to San Bernardino for good (InfoNegocios (opens in a new tab)). If you stay through winter, set up your own plan: a regular doctor nearby, the number of your barrio's health post and a tested route to the regional hospital. In summer, book check-ups on weekdays: long weekends bring long queues at the Ecovía toll (Luque–San Bernardino) and the km 48 PY02 toll in Ypacaraí (Hoy (opens in a new tab)).
Lake, heat and season: what sends people to the doctor
- The lake: MADES approved San Bernardino's beach for recreation but warned that Lake Ypacaraí is not fit for swimming because of cyanobacteria (ABC Color (opens in a new tab)). These algae are dangerous to people and animals (Última Hora (opens in a new tab)). If someone went in and develops irritation, vomiting or feels unwell, see a doctor and mention the lake.
- The heat: the Ministry of Health advises exercising before 10 a.m. or after 5 p.m. and drinking plenty of water (MSPBS (opens in a new tab)). Severe dizziness, confusion or chest pain is an emergency.
- Kids on holiday: bring the vaccination card and regular medicines; in season, appointments fill fast.
